WSIB formularies list

A formulary is a list of therapeutic products available to people with work-related injury and illness. We have developed formularies that are specifically related to types of work-related injuries and illnesses and thus, we do not have a general formulary. A person may have access to one or multiple WSIB formularies. If someone’s diagnosis does not easily fit one of the listed formularies, entitlement will be assigned on a case-by-case basis. 

The following is a list of WSIB formularies and related descriptions. People with a workplace injury or illness who are registered for online services can view their formulary assignment in the Health Care - Medication section. If your formulary assignment is not clear, you can send a message in the online services or call us for more information.

For musculoskeletal injuries such as muscle sprains, strains, orthopaedic trauma

Central Nervous System and Peripheral Nervous System Injury/Disorder

Facial/EENT Injury

04WS

For injuries to the skull or face. It may also involve the eye, ear, nose or oral cavity

Burn

05WS

For injuries to tissues of the body caused by hot objects or flames, electricity, chemicals, radiation or gases. This formulary gives access to select opioid medications with morphine equivalent dose (MED) less than 90.

Respiratory Injury/Disorder

09WS

For injuries due to inhalation of gases or particulate matter or allergens. For example, occupational silicosis, asbestosis or asthma

Cardiovascular Injury/Disorder

14WS

For an occupational aggravation of a pre-existing cardiovascular condition or a traumatic incident or cardiovascular disease or injury due to occupational exposures

Skin Injury/Disorder

15WS

For conditions of the skin caused by occupational exposure. For example, contact dermatitis, allergic dermatitis

Cancer

For neoplasms that are determined to be caused by occupational exposures. This formulary gives access to select opioid medications with morphine equivalent dose (MED) less than 90

Psychotraumatic Disorder

For psychological conditions, disabilities or impairments caused by psychotraumatic work or related incidents. For example, post-traumatic stress disorder

Chronic Pain Disability

Reserved for claims with chronic pain disability entitlement

Initial

25WS

Allowed claims are assigned to this formulary during the acute phase following injury (0-3 months). This formulary gives access to select opioid medications with morphine equivalent dose (MED) less than 90

Serious Injury

27WS

Reserved for claims assigned to the Serious Injury Program (SIP). This formulary gives access to select opioid medications with morphine equivalent dose (MED) less than 90
