Gather the following information to report your employee’s work-related injury, illness, exposure or fatality online.
Injury or illness information
- Provide injury or illness details, including date and time. If you are reporting a mental stress or psycholgical injury, the approximate start date is okay.
- Tell us where the injury or illness happened (e.g. construction site, customer site, in-flight, parking lot, temporary agency location).
Business details
- Confirm your account or firm number, legal business name, North American Industry Classification System (NAICS), trade name, email address and phone number.
Employee details and work history
- Provide your employee’s contact information including address, phone number, date of birth and Social Insurance Number (SIN)
- Give the date your employee started working for you and how long they worked with your business.
- Provide immediate supervisors name and contact information.
- Include the union name and number, if applicable (not mandatory).
Health care
- Share details about your employee’s visit to a health care professional and their contact information.
- If your employee has not seen a health care professional yet, provide the date of upcoming appointment.
Lost time and earnings
- Give details about missed worked, reduced hours or any loss of pay.
- Tell us about any temporary or permanent changes to job duties, hours, or environment.
- Provide contact information of someone we can speak with about any time your employee missed from work, modified work or return to work.
- Give the date of your employee’s last working day before injury or illness and expected date of return to work, if you know it.
- Share details about your employee’s regular work schedule and rate of pay.
- Let us know about any absences from work for vacation or illness in the four weeks before the injury.
Supporting documents
- Submit records of employment and payment, relevant information, records or documentation about the workplace injury or illness.
After you enter the report type, injury or illness details and employer details, you will get a reference number. After you receive the reference number, you can save your report and come back to it later if you need to.
