Make a request

Under the Freedom of Information and Protection of Privacy Act (FIPPA), every person has the right to request access to a record unless it falls within one of the exemptions described in the Act.

Requests may be submitted by mail, in person, or online.

FIPPA prescribes a user-pay principle and requires the person making the record request to pay the fees outlined in the Regulations of the Act. Once the request form is submitted, an FOI coordinator will provide you with an estimate of any additional fee amounts over $25.

You can fill out and print the FOI request form and mail or deliver it together with a $5.00 cheque/money order, payable to the Workplace Safety and Insurance Board:

To submit Freedom of Information request by mail or in person, fill out this request form, print it (use the browser's print function or the keys Ctrl P) , and mail or hand deliver it, along with a $5.00 application fee (in cash or made payable to the WSIB) to:
