Home care services program 

The home care services program supports people with a work-related injury or illness to:

  • transition from the hospital back to their home
  • manage acute and chronic injuries and illnesses and the risk of complications and additional hospitalizations
  • function independently or with continued supports while maintaining a good quality of life  

The home care services program offers a network of contracted health care providers across the province so people who need it will have increased access to home care providers in their communities.

Key components of the program

The program supports people with a wide range of injuries and illnesses including acute, chronic and end-of-life. It uses a bundled care model that allows for flexibility in the type and duration of services provided to fit people’s unique care and recovery needs.

Services offered through the program include:

  • Personal support work that helps people with personal care needs such as dressing, bathing, grooming and more
  • Specialized nursing such as wound, ostomy and continence care and palliative care
  • Rehabilitation provided by occupational therapists or physiotherapists to remove barriers to accessing clinic-based rehab services.
