What are the fees for Health Professional’s Report for Occupational Mental Stress (Form CMS8)?
The WSIB will pay a fee of $85 to physicians and nurse practitioners who complete this form. An electronic version of the form will be available in 2018.
Who can fill out the Health Professional’s Report for Occupational Mental Stress (Form CMS8)?
Only physicians and nurse practitioners can use a Form CMS8. Physicians and nurse practitioners should fill out the form when a patient comes to them with a work-related mental stress disorder or condition.
Can a Health Professional's Report (Form 8) and a Health Professional’s Report for Occupational Mental Stress (Form CMS8) be submitted concurrently on the same day?
You should submit a Health Professional’s Report for Occupational Mental Stress (Form CMS8) to report a patient’s occupational mental stress disorder or condition. If there is a concurrent physical injury or illness, you can complete a Health Professional's Report (Form 8) on the same day.
Are there instructions with the Health Professional’s Report for Occupational Mental Stress (Form CMS8)?
Yes, the form has a cover sheet with instructions. Please complete the form, have your patient sign it and provide them with a copy of only the last page. The last page of this form describes functional abilities and return-to-work information that will help your patient get back to work sooner.
Does the Health Professional's Report (Form 8) or the Health Professional’s Report for Occupational Mental Stress (Form CMS8) replace the Functional Abilities Form?
No. Your patient or their employer can still request a Functional Abilities Form (FAF) on follow-up visits. We do not pay for Functional Abilities Forms that are submitted with a Health Professional's Report (Form 8) or a Health Professional’s Report for Occupational Mental Stress (Form CMS8) on a patient’s initial visit.
When do I complete the Functional Abilities Form?
A Functional Abilities Form (FAF) can only be completed when a request is made by a patient or their employer.
Can I use my own version of the Health Professional’s Report for Occupational Mental Stress (Form CMS8)?
No. Health professionals must only complete the approved WSIB form. The version on our website can be filled out online, saved to file or saved with your keystone information on your computer to make filling it out faster and easier.
