Assessment and treatment

Weeks one to four – minimum three visits must be completed from the date of first assessment/visit (includes visit for initial assessment)

POC must be initiated within six weeks from date of injury Initial Assessment Report – complete and submit to the WSIB within two business days

Weeks five to six – minimum three visits must be completed

To be completed whenever the patient is discharged

Must be submitted within two days of patient discharge from Phase 1 or 2
